The Contractor agrees to provide mental health therapy services as described below:
The Contractor shall provide mental health therapy services including, but not limited to, individual, group, family, and couples therapy, conducted under the highest professional standards and in compliance with all applicable laws and ethical guidelines.
Responsible for assessing, diagnosing, planning, and implementing therapeutic interventions for clients, maintaining confidentiality, and obtaining informed consent for treatment.
Services must be conducted professionally, adhering to the ethical guidelines of (relevant professional body).
Ensuring continuity of care for clients, including timely documentation and coordination with other health care professionals as necessary.

Providing services during agreed times and locations and maintaining reasonable availability for client needs.
Collaborate with the Private Practice's staff and other professionals for client benefit and participate in regular meetings or communications regarding client care.
The Private Practice shall compensate the Contractor as outlined below:
The Contractor shall be compensated at an (hourly/session-based/project-based) rate for services rendered. The specific services and their corresponding rates are as follows: ($X per hour/session or a fixed amount for a particular project).
The Contractor and the Private Practice will mutually agree upon the specific times the services will be provided. These agreed times are to be documented below:

The Contractor shall submit invoices to the Private Practice on a (weekly/monthly/bi-monthly) basis for services provided during these agreed-upon times. Immediate reporting to the Private Practice is required for any suspected or actual confidentiality breaches.
